Facelift Vs. Facial Lipo: Do I Need Both?

Facial rejuvenation is one of the most commonly requested surgical procedures among both men and women. However, the main worry patients have is looking “pulled” or “overdone” which represents a surgical failure. Great plastic surgery whispers, it doesn’t shout! The key to optimal rejuvenation is a natural, graceful, balanced appearance. Through advanced surgical techniques and artistic application of those methods, patients can achieve rejuvenation without telltale signs of surgery.

In an effort to avoid a facelift, many patients request facial liposuction as an alternative. While facial liposuction is a powerful tool, it is only used in a small percentage of patients undergoing facial surgery. Today, we’ll take a closer look at both procedures to help you get an idea about when facial liposuction is a reasonable alternative to a facelift (Spoiler alert – facial liposuction is rarely an alternative to a facelift.) At the time of your initial consult, we’ll discuss each of these options at length before customizing a treatment plan.

Facelift Procedures

Facelift procedures primarily address concerns on the mid-to-lower face. Facial aging involves skin changes – sag, loss of elasticity, and textural changes. It also involves changes in facial volume – loss and repositioning of facial fat.  During the facelift procedure, all of these issues are addressed. Skin and muscles that have begun to sag are tightened and re-repositioned. The artistry of great facelift surgery is understanding when and how far the surgeon can push tightening and repositioning as a method without creating the overdone look that all patients fear. A lot can be accomplished with the lift – as long as one uses the advanced two layer SMAS technique – but for many patients, it is essential to address volume as well.

Patients with round full faces often have the young “baby face” look.  They don’t need to be concerned about volume loss, and these are the patients who, on occasions, will benefit from very limited fat removal – be it with facial liposuction or removal of fat directly through surgical thinning of fat pads.

More often, if there is adequate fat in the face but it has sagged, the SMAS lift allows me to reposition the fat.  Moving the fat from where it has slid (like the nasolabial fold, or the jowl) to where it was lost (the malar eminence, or the cheek hollow) is not the same as reducing the nasolabial fold or jowl with liposuction.  Doing that would exacerbate the aging – making the face look even more hollow. So, the takeaway message is that fat is only reduced in the round full face – in the majority of cases, it is repositioned to where it was originally situated.

At your initial consultation, it is very helpful to see old photos.  Bringing photos of yourself in your younger years, where your face had a more youthful appearance, is a great tool to explain what needs to be done.  I can demonstrate where the fat was – where it’s gone – and if it’s missing or just repositioned.

Note: Aging often involves LOSS of facial fat.  If fat is lost, it often must be replaced to get the very best results.  Fat loss is volume loss. Correction involves volume replacement – either using your own fat as a transplant or using fillers like Juvederm® or Restylane®. And that’s a subject for yet another blog!

Are There Any Scars With a Facelift?

Facelift surgery can be a game-changer for many men and women. They’re able to achieve a younger, more youthful contoured facial appearance with a single procedure. Today, we’re going to address one of the most commonly asked questions “Are there any scars with a facelift?” Understandably, scarring can be a valid concern with any surgery, but I’ve got good news for you!  

Through advanced surgical techniques, patients are able to get the results they want with almost no visible signs that they’ve had any surgery at all. You’ll achieve a refreshed, naturally-younger looking you with minimal downtime, and generally virtually invisible scars.

Where Are Incisions Made?

Reality check – there is no such thing as a facelift without incisions.  To recontour and reposition structures in the face that have sagged and drooped, we have to make incisions in the skin.  While I perform the latest techniques, including minimal incision facelifts, your particular anatomy and desires determine what has to be done – and what has to be done always involves some skin incisions. The goal is to make those incisions invisible to both you and your friends and loved ones (as well as those jealous people you also know!!!)

During facelift procedures, incisions are typically made in well-hidden locations. In most cases, I will create an incision along the base of the sideburn, beveled so that when your hair grows it will grow through the incision and conceal it.  I then move just in front of the ear, in the natural crease and shadow line where the upper ear touches the cheek. I continue in that crease line to the tragus – the little flap of skin and cartilage that covers the ear canal.  At that point, I go inside the ear, then just in front of the earlobe where it connects to the cheek. I then turn around and under the earlobe into the crease in the back of behind the ear up to the hair then either into the hair-bearing scalp or along the hairline behind your ear.  The purpose of hugging the ear so closely is to take advantage of natural shadows and creases.  That way, any residual scar that is seen will appear to be a crease line, not a scar!

When a neck lift is performed with the facelift, a small incision is made under the chin – this generally appears as a natural skin crease.  No additional skin incisions are necessary for the neck lift.

Concealing the incisions in shadow and crease lines is the first step. Gentle handling of the skin during surgery is critical, and obsessive attention to suturing technique is also necessary to make your incisions virtually invisible. Skin staples, which were developed in the 1970s, are a quick way to close skin incisions but are not a substitute for artistic suturing using sutures that are often thinner than a human hair.  I never ever use staples to close the skin… and I never will!  

When done correctly by a skilled surgeon, facelift scars are almost non-existent. I work in such a way as to meticulously open and close my incisions to minimize tissue disruption. This specialized technique ensures an optimal outcome with minimal swelling, bruising, post-surgical bleeding, or scars.

What Can You Do to Minimize Scars?

With facelift procedures, you do not need to actively do anything – my techniques produce the thinnest possible incisions, and the two-layer SMAS technique eliminates the tension on the incisions seen with most single layer techniques.  The SMAS layer, which is under the skin, absorbs the tension.  Smoking (including nicotine patches), excessive alcohol, and sun are all enemies of wound healing, and these must be avoided.

Your recovery plan typically includes avoiding smoking of any type, alcohol, and sun exposure. Furthermore, pushing yourself too hard initially can prolong the healing process and widen scars. In the majority of cases, I do not recommend any of the available scar creams, Vitamin E, or Aloe.  

When Will Scars Go Away?

Incisions are typically red, then gradually fade.  Scars mature with time, and if your incisions are red or slightly raised, that will generally disappear with time. It can take upwards of 6-12 months for scars to diminish in size and color completely, but typically, after several weeks, scars are barely noticeable. Makeup can be applied about the time you get back into your normal routine to minimize the appearance of surgical scars. Most patients find the minimal scars well-worth the final outcome of their facelift procedure.

I will provide you with guidance throughout recovery for achieving an optimal outcome with little to no scarring.

Am I Too Young for a Facelift

Everyone ages differently. Genetics, sun damage, and other environmental factors all play a role in when the underlying foundation of our face starts to change. That being said, there is no set age or simple answer to the question of when to do your facelift.  Instead, when evaluating clients, I focus more on the severity of their aging condition, skin elasticity, sag, and droop.  This is much more important than age as a determining factor.

Droopy or Sunken Cheeks

Volume loss can cause our cheeks to slowly fall over time. While a loss of volume is typically associated with the natural aging process, hormonal changes and weight fluctuation can also affect how well our mid-face looks. While synthetic fillers can be a good short-term solution for droopy or sunken cheeks, a facelift, with or without fat grafting, remains the gold standard for these concerns.

Excessive Nasolabial Wrinkles and Folds

Repetitive facial expressions and movements can cause deep, dynamic wrinkles around the nose and mouth. Some patients are more susceptible than others and can start to see the formation of nasolabial folds as early as their mid-to-late 30s. Facelift procedures re-drape the skin and tighten underlying muscles for long-term wrinkle reduction.

Loose, Sagging Skin

Our collagen levels can start to drop as early as our mid-20s which thins the skin.  We also begin to lose the elastic fibers in our skin at this time. Excessive sun exposure accelerates this normal process. As this decrease occurs, many men and women see loose sagging skin that wasn’t there earlier in life. As a general rule, patients should have at least a centimeter of pinchable skin laxity on the lower jawline before considering a facelift procedure.  The exception to this rule is when the deeper structures (muscle and deeper fat pads) have sagged, or when substantial volume loss has occurred. In those cases, early facelift procedures, focusing on the deeper structures rather than the skin, are helpful.

Is A Facelift Right for Me?

You may be a good candidate for facelift procedures if you are unhappy with your facial appearance and you’ve considered all of your non-surgical options, or if the non-surgical options are no longer providing the results you want. You should be an otherwise healthy non-smoking adult with realistic expectations about what the procedure can do for you.  A well trained plastic surgeon who specializes in facial rejuvenation surgery can give you an accurate assessment of your suitability for facial surgery. In my practice, I turn away about 30% of all patients who come in for a consultation.  For many of the younger patients, that “rejection” is temporary. I tell them to come back annually for a no charge follow up a consultation. At some point, we both determine that now is the right time for surgery. I learn about their skin, their aging process, and their expectation – and we build trust – so that when the time is right, we both feel comfortable moving ahead.  

The best way to determine if a facelift is right for you is through an in-person consultation and evaluation with a skilled facial surgeon such as myself. We may decide that you’re better suited for a mini facelift or a minimally-invasive procedure rather than a full facelift. I’ll work closely with you to help restore your natural beauty so you can feel confident in your own skin once again.
