With cosmetic breast surgery, I have three options – I can make your breasts larger (breast augmentation or augmentation mammoplasty), I can make your breasts smaller (breast reduction or reduction mammoplasty), and I can lift your breasts (breast lift or mastopexy).  I can also do combinations – breast reduction and lift, or breast augmentation and lift.

A breast augmentation makes your breast larger by placing an implant behind your natural breast – the final size is the combination of the implant I place plus your natural breast tissue. A breast reduction makes your breast smaller by reducing the breast glandular tissue and breast fat, as well as removing extra skin.

What is a Breast Lift?

A breast lift usually involves reshaping the breast tissue into a more attractive form and reducing a small amount of skin. Generally, there should be almost no reduction in the breast size, but very commonly, patients tell me that their breasts appear smaller when they are naked. In a bra, you’ll look the same.

Why Do Lifted Breasts Appear Smaller After the Lift?

Easy answer! Breasts that are droopy also tend to be wide – it’s the pancake effect.  From the front view, when you are naked, the breast is wide.  You don’t notice the lack of forward projection when you’re looking from the front.

When I do the breast lift, I reshape the pancake-shaped breast into a nicer mound, which provides more forward projection– and that generally means less width, sometimes referred to as less “side boob.” Think of a ball of clay. If you put the ball on your chest, it projects outward and the width is relatively small.  If you crush the clay ball against your chest, the forward projection decreases, but the width increases. So – naked your lifted breast will seem smaller, but in a bra it will look the same.

Walnut Creek Deep Plane Facelift

The first facelift was performed in 1901 by a German surgeon in Berlin.  An elderly Polish aristocrat asked him to “lift my cheeks.” He did the operation by making incisions in front of and behind the ears. The lift involved pulling on the skin, and the removing the excess. While the technique changed somewhat over the next 70 years, the “skin” facelift was the procedure for facial rejuvenation until the early 1970’s. 

Tord Skoog, a Swedish plastic surgeon, was frustrated by the results of skin facelifts. Skin stretches, so when the skin lift was performed conservatively, the skin stretched quickly, resulting in a loss of the benefit. More aggressive pulls on the skin lasted longer, but the skin became stretched, and unnatural lines appeared – the famous “wind tunnel” effect. Skoog was performing a facelift one day and he lifted the skin but went deeper – he lifted the skin and the underlying muscle/fascia layer that overlies the parotid saliva gland. He called this new layer the pre-parotid fascia, and in 1972 the “deep plane” lift was born.

Four years later, two French plastic surgery residents, Mitz and Peyronie, went to the anatomy lab and began to dissect cadavers. They determined that the “pre-parotid fascia” was actually a distinct layer separate from the parotid gland, and they called it the “Superficial Musculo-Aponeurotic System” or SMAS.

The deep plane is considered the plane below the SMAS. A true deep plane facelift does very limited dissection below the skin layer – just enough to get to the SMAS layer – and the remainder of the dissection is below the SMAS. A “two-layer” facelift lifts the skin and the SMAS as separate layers.  First, the skin is lifted, then the SMAS is lifted.

Many surgeons say “I do a deep plane facelift” or “I do a two-layer facelift” and they insist that their technique is the best. The reality is that all faces age differently – in some patients, a deep plane technique is best and in others, a two-layer technique is best. One thing is clear – the “best” plastic surgeons know how to do both techniques and use the technique that is best for each given patient. And that’s what I do!

With most of my outpatient surgeries, patients receive either sedation anesthesia or general anesthesia. While I also use local anesthesia during your procedure, sedation or general anesthesia reduces anxiety and pain. Patients who have sedation may have some awareness of the surgery, while those under general anesthesia have none. With both procedures, after the surgery patients will be somewhat groggy for a period of hours. Usually, the mental grogginess clears by the next day and patients feel more like themselves.

Because of the grogginess that occurs after either anesthesia type, patients should not operate dangerous machinery, nor should they drive until the following day at a minimum. I don’t want them to injure themselves or anyone else!

Also, because of the grogginess, I am not comfortable allowing my patients to go home in a cab with a stranger. Patients accompanied by a friend or family member can take a cab home since they will not be alone. Likewise, patients leaving with a hired nurse can also take a cab. Patients in a cab alone with an unknown person (the cab or Uber/Lyft driver) is not acceptable. A driver may not show the appropriate concern for a rider’s well being, and could, potentially, take advantage of someone’s slightly unclear mental state. As a general rule – the hospitals and surgery centers that I work with have rules requiring patients to be accompanied by friend or family member, for the same reasons.

Depressed or sunken cheeks can have an adverse effect on how you perceive yourself. I have both men and women who come into my office and say they are unhappy with their facial structure. Filling in your cheeks can give you a better profile and a more balanced overall appearance. Patients who are looking to add long-term volume to their features yet aren’t ready for a full facelift, may want to consider a fat transfer procedure.

How It Works

Fat grafting (or fat transfer) is a minimally-invasive in-office procedure that’s done in a simple 3-step visit. First, a small amount of autologous fat (fat from your own body) is removed from a predetermined area (typically the abdomen, thighs, flanks, upper arms, or hips) via liposuction. Next, one of our skilled technicians will process and cleanse the fat while you wait. Lastly, the fat is injected under the cheekbone to give you a fuller-looking facial appearance. Fat transfer may be a bit more time-consuming to perform than pre-packaged dermal fillers, but the results are much longer lasting.

Fat Grafting Results

How your body reacts to fat transfer plays a significant role in the outcome of your procedure. Because fat is living tissue, it’s naturally reabsorbed into your body and develops a new blood supply. Fat cells are incredibly temperamental. During your fat grafting procedure, I add extra volume to compensate for fat cells that may not survive the injection process. Final results can be seen in as little as 3-months. Fat transfer is considered a permanent procedure, however, weight gain (or loss) can affect the outcome.

Am I a Good Candidate?

You may be right for fat grafting if you would like to enhance the appearance of your facial features and have a suitable donor site. Some patients may be a better fit for dermal fillers such as Sculptra©, Restylane©, or Juvederm© products.
