In my last blog, I discussed breast implant shapes to explain how I customize my choice of implant for you.  In this blog, I’ll discuss implant size.

Breast implant size is a highly personal decision.  As your surgeon, I will work with you to determine the size that is best for you – it is a collaborative process.  Your overall body shape and size, your chest wall shape and size, the elasticity of your skin, and the incision and implant type you choose determine a range of what I can use during surgery.  The range is generally quite large, and once I examine you and determine a safe range, you will be in charge of deciding the final size.

How Do You Determine What Size You Want to be After Surgery?

There is no perfect, one size fits all method to make the decision.  The decision should not be rushed, and you should do some (fun) homework before you come to my office for the sizing visit.  At the sizing visit, we will finalize the implant size choice.

Before the sizing visit, I recommend the following:

First, collect photos of full dressed (not bathing suits or naked) women in magazines or online.  Focus on photos that contain women who have a body shape similar to yours. Bring photos to my office that show sizes you like, and those you don’t like. Put a sticky note on the picture, and write what you like and don’t like on each photo.

Second, do the same thing with photos of women in bathing suits or naked. Bring those photos to the sizing visit as well.

Third, purchase a bra, two pounds of white rice, and zip close plastic baggies (like Ziploc™ Baggies). The bra should have an underwire and a stretchy cup – not a formed cup and no padding. The underwire should fit your chest and the cup should be too large for your natural breasts. Put the bra on and place different amounts of rice in the baggies, then put the baggies in your bra. Keep trying different amounts of rice until you find a size that you like.

It’s helpful to put on a tank top over the bra and rice and to wear leggings or tight jeans.  You can also try on different clothing outfits that reflect your lifestyle.  When you find an amount of rice that you like, tape the baggie shut (so it won’t leak) and bring the filled baggie in with you to your sizing visit.

If you have different size breasts (very common), adjust the volume of rice in each baggie and mark the baggies right or left.  Bring them to the sizing visit.

Finally, choose three or four different tops that you commonly wear.  Choose tops with and without buttons, both loose and tight.  The plainer the top the better – best if they are solid colors without designs, pleats, or pleats.  Bring them to the sizing visit.

At the sizing visit, I will review the photos and the rice baggies with you, and then repeat the rice test with actual silicone gel implants. I’ll have you try multiple implants and your different tops with each implant. Each time we try on a different implant, we’ll discuss the look and answer any questions you may have.  It’s okay to bring a friend or family member to the sizing visit.

Why do I ask you to do the pre-sizing visit photo collection and rice test?  At the actual sizing visit you will already have an idea of what you’d like to look like after the breast enlargement surgery, and we will be able to focus on fine-tuning your decision.  However, if you don’t have time to do the pre-sizing visit tasks (photos and rice) don’t worry! We’ll figure it out!

In my last blog, I discussed the textures of breast implants to help explain how I customize my choice of implant for you. In this blog, I’ll be discussing implant shape.

For many years implants were only available in one shape – round.  Not round like a ball, but round like the top of a muffin.  If you put the implant on a table and look at it from the top down, it is round in shape.  If you look at it from the side, the implant is flat on the bottom and curved over the top.  Implants are placed in the breast with the flat surface against your ribs and the curved side towards your breast.

Manufacturers have tried for years to switch plastic surgeons to teardrop shaped implants, claiming that the ideal breast is naturally teardrop shaped.  While I can’t argue with that, the question is, does a teardrop shaped implant create a more attractive breast then a round implant?

Think of a plastic bag filled with water: if you hold it at the top of the bag, you’ll notice that the bag takes on a teardrop shape – more water at the bottom of the bag, and less towards the top. That’s what happens with a round implant – due to gravity, it naturally takes a teardrop shape when a woman stands up, but when she is lying on her back, it becomes more round (muffin top) like a natural breast. Round implants act like teardrop implants when a woman is upright – and that’s what you want.

Large studies have been performed where plastic surgeons are shown before and after photos of women with either teardrop shaped implants or round implants, and the surgeons are asked which implants the women have.  The studies have consistently shown that the surgeons can’t determine the implant type – the teardrop and round implants look the same after surgery!

What are the negatives of teardrop shaped implants? Teardrop implants cost approximately twice as much as round implants.  If the teardrop implant rotates after surgery, the breast shape will be negatively impacted. Finally, teardrop-shaped implants have a textured surface which creates certain problems (see my earlier blog on textured vs smooth breast implants).  For these reasons, I rarely recommend teardrop implants.

In previous blogs, I’ve discussed various characteristics of breast implants to explain how I customize my choice of implant for you. In this blog, we’ll discuss implant texture.

The implant surface can be manufactured smooth, or rough like sandpaper, hence the common references of smooth or textured. Once inside your breast, the implant touches your body tissues. Your tissues respond to the implant by producing a thin layer of scar tissue that surrounds the implant, called a scar capsule.  At times, that scar capsule can tighten or contract around the implant, in a process called capsular contracture or “CC”.

CC can cause your breast to feel hard, and to be misshapen, requiring additional surgery to correct.  Implant manufacturers have tried to prevent CC by influencing the interface between the outside of the implant shell and your tissues, so that the scar capsule will not contract.  One of the ways the manufacturers alter the interface is by changing the texture of the outside surface of the implant – the surface that is in contact with your body – offering smooth or textured implants.

The scar capsule around smooth implants is smooth and the scar and implant can slide on each other. The original theory behind using a textured surface to prevent CC was that the scar tissue would form following the ins and outs of the texturing, producing a scar capsule that could stretch like a zig zag stitch in clothing.  With rougher textured implants, the implant is attached to scar capsule, just like a Velcro™ attachment.

Research studies on large numbers of women comparing capsule contracture rates of smooth vs textured implants have shown that the rate of CC is slightly lower when using textured implants. The reduction is not huge, so we have to consider the risks and benefits of smooth vs textured implants when choosing the right implant for you.

As you age, or following pregnancy or weight gain or loss, your breasts may droop. If the implant is fixed to your chest wall, it tends to stay in place and your breast tissue may “fall” off the implant, like a waterfall or a slinky toy.  This produces an unnatural appearance to the breast. On the other hand, when I perform a breast lift along with a breast enlargement, the fixed position of the implant may help to prevent the breast from drooping.  These are considered in choosing the implant type.

With tear drop shaped implants, your breast will look funny if the implant rotates, putting the larger part of the teardrop on the upper portion of your breast.  The Velcro™ effect of textured implants prevents the implants from rotating, so all shaped implants are textured. Round implants don’t have a large and small side, so rotation is not an issue and the orientation is unimportant.

Over the past decade, plastic surgeons have found that there is a rare form of lymphoma that seems to be associated with breast implants, but almost only textured breast implants.  The reason for the association is not known, and fortunately, the cure of the lymphoma generally just involves removing the implant and scar capsule. While there have been a very few cases with smooth implants, the risk is much higher with textured implants.

In the United States, by far smooth implants are more popular than textured.  In my practice, I use both types of implants, but I reserve textured implants for only certain patients.  At your consultation, this will be discussed in detail, and we will jointly reach a decision that’s correct for you.

In a past blog on breast implants, I discussed the different implant manufacturers. In this blog, I’ll discuss the different options for filling implants.

Implants consist of two components – the implant shell and the implant fill material.  Simply, the shell is a container for the fill. The shell is constructed of a silicone type material, is elastic, and will be discussed in detail in another blog. The fill material is either saline (sterile salt water) or silicone gel. Implants can be roughly categorized as saline or gel, based on the fill material.

Saline Implants

Saline implants are provided to the surgeon as an empty shell. A fill tube is attached to the shell and the implant is inserted empty. Once in place, the surgeon injects saline through a fill tube. Once the implant is filled, the tube is removed. There is a valve in the shell that accepts the fill tube, and this valve seals automatically when the fill tube is removed.

Saline implants have a fill range so that minor adjustments in size can be made once the implant is in place – the range is generally about 10%, say 300cc to 330cc.  Also, since saline implants are placed deflated, then inflated, they can be placed through very small incisions. Saline implants have a firmer feel than gel implants and a greater tendency to ripple which can be felt through the skin in thin, smaller breasted women.  Saline implants generally last 10-15 years and then need to be replaced due to leakage.  When they leak, the saline is absorbed by your body and represents no health risk.  Replacement is simple but is performed in an operating room.

Saline implants are less expensive then gel implants and the risk of capsular contracture is lower.  There is no age restriction on the use of saline implants.

Gel Implants

Gel implants are provided to the surgeon as a filled implant and therefore require a longer incision to insert. The gel can be thinner or thicker (more or less cohesive) depending on the specific model of implant chosen.

Gel implants since 2006 are all cohesive, meaning the gel is designed to stick together as a single mass and not ooze away, even if the shell fails.  The thickest gels can be molded into a specific form (teardrop) and will maintain that shape – they are called Form Stable (sometimes called “Gummy Bear” implants.)  As cohesive gel implants are relatively new the lifespan is not yet known.  I anticipate that they will last, on average, 20-25 years or longer.

Gel implants are softer than saline implants and have less tendency to ripple. Gel implants are more expensive than saline, but as they last much longer, are less expensive in the long run. Capsular contracture risk is higher than saline. Gel implants can only be used in women 22 or older, based on FDA restrictions.

Gel implants are much more popular than saline, but saline implants definitely have a place.  The safety profile, other than the risk of capsular contracture, is the same for gel and saline.  No studies have compared the impact of saline vs gel on breastfeeding but the general consensus is that neither impact breastfeeding babies.

In my last blog about breast implants, I provided a brief overview of implant options. In this blog, I’ll discuss the Manufacturers.

As of 2017, there were three major manufacturers of breast implants. Breast implants are controlled by the FDA, just like drugs, and all manufacturers must follow very strict guidelines and practices to be allowed to manufacture and sell breast implants. It is not legal to use implants from non-FDA approved companies, and there must be a “chain of possession” between the manufacturer and the doctor.  Surgeons are not permitted to use implants provided by overseas companies, or to use implants provided to the surgeon by the patient.

All three manufacturers, Allergan, Mentor, and Sientra are large, publicly traded companies. That’s important to you as a consumer, because they have the resources to test and produce high quality products. I work with all three manufacturers, and since I’ve been doing breast augmentation for quite awhile, I have excellent relationships with senior people at all three. This is helpful when there are implant shortages, or when new products with limited availability, are offered. I’ve also done research with these manufacturers.

Breast implants are mature products. They’ve been around for over 50 years, and while there are small differences in the offerings from the different manufacturers, the differences are minor. One manufacturer has no major advantage over the other. Their prices are also very competitive. All manufacturers offer warranties on their implants – the warranties change frequently – but because of the competitive nature of the business, the differences are marginal. You can review the warranties on the individual manufacturer’s website. While warranties change, the warranty in place when you have your surgery is the warranty that the manufacturer is bound by.

Major improvements in implant technology have occurred over the years and these same improvements, with subtle differences, have been adopted by all three manufacturers. At any given time, one manufacturer’s latest upgrade may give it a temporary advantage, but its’ generally short lived.

Take Away Message

No single manufacturer stands out above the others. That said, the subtle differences between the implants may affect you due to your particular needs. The best breast implant surgeons will choose the implant that is correct for you and go to the manufacturer that makes that implant!

Breast augmentation, or augmentation mammoplasty, remains one of the most frequently requested plastic surgery procedures I receive to date. With the newest technology and techniques, breast augmentation has become safer and more natural-looking than ever. Patients that come into my office with realistic expectations have a high satisfaction rate after the procedure. Today I’ll discuss what may or may not make you a good candidate for breast implants.

Is breast augmentation right for me?

Well, that depends. This procedure, like all surgery, requires a good deal of consideration. It is important to ensure that my patients want to do breast surgery for themselves and not for someone else. Breast augmentations are designed to enhance your self-image, not please a boyfriend, spouse or family member.

That being said, breast augmentation can be an empowering procedure for many women. For healthy women who feel that their breasts are too small, implants may be an option. Also, many women come to my office complaining about a change in breast shape, droop of the breasts, or loss of breast volume following pregnancy, severe weight-loss, or aging. These women can also benefit from breast enhancement surgery. Before opting for any surgical procedure, it’s important to familiarize yourself with every aspect so you can make an informed decision.

What can I expect from my procedure?

Breast augmentation, overall, is a relatively easy recovery.  There is some discomfort for the first few days, and we provide pain medication.  Most patients return to work in about 4 days, 7 days if your work is more physical.  I do place restrictions on exercise for a week, and high impact activities, like running, for 3 weeks.  You may experience swelling, tenderness, soreness, and bruising. Full recovery varies by patient.  Implants may be slightly high for the first six weeks until they drop into place.  For that reason I recommend putting off lingerie shopping for the first six weeks.  Sexual activity is fine after the first week, but be gentle!  Depending on the implants chosen, implant replacement surgery may be necessary.  For saline implants, replacement is commonly required after about 15 years, for silicone I anticipate 30 years, but this very variable.

Are there alternatives to breast augmentation?

Yes. Breast augmentation surgery isn’t the correct procedure for every patient. You may be a better candidate for breast mastopexy (breast lift) or a combination of lift and augmentation.  Fat grafting as an alternative to implants is an option, but the amount of enlargement is very limited, and multiple procedures to obtain your desired result may be necessary.
